Peringatan Escitalopram

Salah satu kontraindikasi escitalopram adalah ada riwayat hipersensitivitas pada escitalopram atau citalopram. Kontraindikasi

Peringatan

Impact of the antidepressant citalopram

Background
Over the last two decades, there has been a constant increase in prescription rates of antidepressants. In parallel, neuroactive pharmaceuticals are making their way into aquatic environments at increasing concentrations. Among the antidepressants detected in the environment citalopram, a selective serotonin reuptake inhibitor, is one of the most commonly found. Given citalopram is specifically designed to alter mood and behaviour in humans, there is growing concern it can adversely affect the behaviour on non-target wildlife

Methods
In our study, brown trout were exposed to citalopram (nominal concentrations: 1, 10, 100, 1000 µg/L) in two different life stages. Larvae were exposed at 7 and 11 °C from the eyed ova stage until 8 weeks post yolk sac consumption, and juvenile brown trout were exposed for 4 weeks at 7 °C. At both stages we measured mortality, weight, length, tissue citalopram concentration, behaviour during exposure and behaviour in a stressfull environment. For brown trout larvae additionally hatching rate and heart rate, and for juvenile brown trout the tissue cortisol concentration were assessed.

Results
During the exposure, both larvae and juvenile fish exposed to the highest test concentration of citalopram (1 mg/L) had higher swimming activity and spent longer in the upper part of the aquaria compared to control fish, which is an indicator for decreased anxiety. Most probably due to the higher swimming activity during the exposure, the juveniles and larvae exposed to 1 mg/L citalopram showed decreased weight and length. Additionally, in a stressful artificial swimming measurement device, brown trout larvae displayed the anxiolytic effect of the antidepressant by reduced swimming activity during this stress situation, already at concentrations of 100 µg/L citalopram. Chemical analysis of the tissue revealed rising citalopram tissue concentrations with rising exposure concentrations. Tissue concentrations were 10 times higher in juvenile fish compared to brown trout larvae. Fish plasma concentrations were calculated, which exceeded human therapeutic levels for the highest exposure concentration, matching the behavioural results. Developmental parameters like hatching rate and heart rate, as well as mortality and tissue cortisol content were unaffected by the antidepressant. Overall, we could trace the pharmacological mode of action of the antidepressant citalopram in the non-target organism brown trout in two different life stages.

Gene expression biomarkers of response to citalopram

Abstract
There is significant variability in antidepressant treatment outcome, with ∼30–40% of patients with major depressive disorder (MDD) not presenting with adequate response even following several trials. To identify potential biomarkers of response, we investigated peripheral gene expression patterns of response to antidepressant treatment in MDD. We did this using Affymetrix HG-U133 Plus2 microarrays in blood samples, from untreated individuals with MDD (N=63) ascertained at a community outpatient clinic, pre plus post 8-week treatment with citalopram, plus used a regression type to assess the impact of gene expression differences on antidepressant response. We carried out technical validation of significant probesets by quantitative reverse transcriptase PCR plus conducted central nervous system follow-up of the most significant result in post-mortem brain samples from 15 subjects who died during a current MDD episode plus 11 sudden-death controls. A keseluruhan of 32 probesets were differentially expressed according to response to citalopram treatment following false discovery rate correction. Interferon regulatory factor 7 (IRF7) was the most significant differentially expressed gene plus its expression was upregulated by citalopram treatment in individuals who responded to treatment. We found these results to be concordant with our observation of decreased expression of IRF7 in the prefrontal cortex of MDDs with negative toxicological evidence for antidepressant treatment at the time of death. These findings point to IRF7 as a gene of interest in studies investigating genomic factors associated with antidepressant response.

Cardiovascular Considerations in Antidepressant Citalopram

There is a definite correlation between cardiovascular diseases and depressive disorders. Nevertheless, many aspects of this association have yet to be fully elucidated. Up to half of coronary artery disease patients are liable to suffer from some depressive symptoms, with approximately 20% receiving a diagnosis of major depressive disorders. Pharmacotherapy is a key faktor in the management of major depression, not least in patients with chronic diseases who are likely to fail to show proper compliance and response to non-pharmacological interventions. Antidepressants are not deemed completely safe. Indeed, numerous side effects have been reported with the administration of antidepressants, among which cardiovascular adverse events are of paramount importance owing to their disabling and life-threatening nature. We aimed to re-examine some of the salient issues in antidepressant therapy vis-à-vis cardiovascular considerations, which should be taken into akun when prescribing such medications.

Introduction
A definite correlation exists between cardiovascular diseases (CVD) and depressive disorders, yet many aspects of this relationship have remained challenging. Up to 50% of patients with coronary artery disease (CAD) suffer from some depressive symptoms and approximately 20% of them have a diagnosis of major depressive disorder (MDD). On the other hand, it has been demonstrated that depressed patients are more vulnerable to experience myocardial infarction even after controlling their pure cardiovascular risk factors. Moreover, significantly higher risk of mortality due to cardiovascular events has been reported in patients suffering from psychiatric disorders such as depression. The diagnosis of depression can be difficult in people with CVD as depressive symptoms such as fatigue and low energy are common in people with CVD and may also be a side effect of some drugs used to treat CVD such as beta blockers. The diagnosis may be further complicated in such patients by their responses to their disease, which may include denial, avoidance, withdrawal, and anxiety.

According to the World Health Organization (WHO) reports, ischemic heart disease is now the leading cause of death worldwide and by 2030, depression will have the greatest burden among diseases in terms of years lost due to disability. Besides impairing patients’ quality of life, depression has profound negative effects on the long-term prognosis of individuals with any style of cardiovascular disorder. These facts further underscore the importance of the proper treatment of depression in patients with CVD.

Abstract
Dear Doctor Letters (DDLs, Direct Healthcare Professional Communications) from 2011 provided guidance regarding QTcprolonging efects with risk of torsade de pointes during treatment with citalopram and escitalopram. This study examines
the DDLs’ efects on prescription behavior. Data from 8842 inpatients treated with citalopram or escitalopram with a primary
diagnosis of major depressive disorder (MDD) were derived from a European pharmacovigilance study (Arzneimittelsicherheit in der Psychiatrie, AMSP) from 2001 to 2017. It was examined to what extent new maximum doses were adhered
to and newly contraindicated combinations with QTc-prolonging drugs were avoided. In addition, the prescriptions of psychotropic drugs before and after DDLs were compared in all 43,480 inpatients with MDD in the information set. The proportion of
patients dosed above the new limit decreased from 8 to 1% in patients≤65 years and from 46 to 23% in patients>65 years
old for citalopram versus 14–5% and 47–31% for escitalopram. Combinations of es-/citalopram with other QTc-prolonging
psychotropic drugs reduced only insignifcantly (from 35.9 to 30.9%). However, the proportion of patients with doses of
quetiapine>150 mg/day substantially decreased within the combinations of quetiapine and es-/citalopram (from 53 to 35%).
After the DDLs, prescription of citalopram decreased and of sertraline increased. The DDLs’ recommendations were not
entirely adhered to, particularly in the elderly and concerning combination treatments. This might partly be due to therapeutic
requirements of the included population. Ofcial warnings should consider clinical needs.

DDLs has been challenged , in part due to insufcient
quality of warning instructions in DDLs . Furthermore,
a Dutch study found that DDLs regarding the risk of hospitalization are less efective than DDLs related to the risk of
death or disability . In addition, advisories recommending
closer monitoring of patients appear not to have a large and
sustained impact on clinical practice . Since prescription
behavior is infuenced by several factors and not only by
DDLs, it is difcult to attribute potential changes in prescription behavior directly to specifc warning messages .
In 2011, DDLs (called “Rote Hand-Briefe”, RHB, in
Germany) regarding citalopram and escitalopram were sent
out by Lundbeck in the three German-speaking countries
(Germany, Austria and Switzerland; . Citalopram
and its active S-isomer escitalopram are selective serotonin
reuptake inhibitors (SSRIs) and are commonly used in the
treatment of major depressive disorder (MDD) . Due to
their efcacy and tolerability, they are widely used in adult
as well as in geriatric populations .
The DDLs recommended a reduction of the maximum
daily dose of citalopram and escitalopram due to the risk of
QTc prolongation. In addition, the DDLs advised against the
combination of es-/citalopram with other potentially QTcprolonging drugs. Similar instructions were issued by the
US Food and Drug Administration (FDA) as well as
by the European Medicines Agency (EMA) . The FDA
further recommended electrocardiogram (ECG) monitoring
in patients taking citalopram who are at particular risk of
QTc prolongation such as older patients, females, as
well as patients with a history of heart disease, hypokalemia
or hypomagnesemia

Citalopram

Het Nederlands Bijwerkingen Centrum Lareb ontving meerdere meldingen van priapisme bij het gebruik van citalopram (Cipramil®). Priapisme is een langdurig aanhoudende erectie ten gevolge van een verstoorde hemodynamiek. Bij niet tijdig ingrijpen kan dit leiden tot blijvende schade met functieverlies tot gevolg. Priapisme is een bekende, ernstige bijwerking van verschillende SSRI’s. In de productinformatie van citalopram, in 2006 na paroxetine de meest voorgeschreven SSRI, wordt deze bijwerking echter niet vermeld.

Casuïstiek. Lareb ontving tot 1 augustus 2007 vier meldingen van priapisme bij het gebruik van citalopram. Het betreft mannen in de leeftijd van 32 tot 60 jaar die niet bekend waren met eerdere episoden van priapisme of met risicofactoren voor deze aandoening. Twee patiënten gebruikten alleen citalopram. Patiënt A startte met het gebruik van citalopram in een dosis van 10 milligram. Bij patiënt B werd de dosis verhoogd van 10 naar 20 milligram per dag. Bij beide patiënten trad binnen een dag een langdurige, pijnlijke erectie op. Na staken van de medicatie trad bij beiden volledig herstel op. Patiënt C en D gebruikten comedicatie waarvan priapisme een bekende bijwerking is en dus mogelijk een rol speelde bij het ontstaan van priapisme. Patiënt C gebruikte quetiapine en bij patiënt D werd citalopram toegevoegd aan sertraline. Desalniettemin had geen van deze patiënten eerder klachten van priapisme gehad. Kort nadat citalopram was toegevoegd, trad priapisme op dat na staken van citalopram weer verdween. Ingrijpen was bij geen van de patiënten noodzakelijk.
Bij alle vier patiënten bestaat een duidelijke tijdsrelatie tussen het gebruik van citalopram en het optreden van priapisme. Hoewel bij de twee laatste patiënten invloed van de comedicatie niet uitgesloten is, lijkt ook bij hen het gebruik van citalopram een belangrijke rol te spelen.

Literatuur. In de literatuur zijn twee patiënten beschreven met priapisme bij het gebruik van citalopram.3 4 Beide patiënten hadden priapisme in de voorgeschiedenis, waarbij er ook sprake was van gebruik van comedicatie die een rol zou kunnen spelen bij het ontstaan van priapisme. Bij beide patiënten was een spoedingreep noodzakelijk. Bij de eerste patiënt trad een episode van priapisme op na toevoeging van citalopram 40 mg aan risperidon. Na de ingreep en na staken van citalopram en risperidon verminderden de episoden van priapisme in frequentie, duur en intensiteit tot het niveau waarmee de patiënt bekend was. De tweede patiënt betrof een 59-jarige man die een episode van priapisme doormaakte bij gebruik van trazodon, (ook een bekende risicofactor voor priapisme), citalopram 20 mg en tamsulosine. Het priapisme trad op nadat patiënt abusievelijk vier tabletten van 20 mg citalopram innam.
Ten slotte zijn er drie patiëntes beschreven met clitoraal priapisme bij het gebruik van citalopram.

Citalopram zur Behandlung

Patienten, die an einer Alzheimer-Demenz erkrankt sind, leiden häufig unter einer psychomotorischen Unruhe, welche sich in einem gesteigerten Bewegungsdrang, aggressiven
Verhaltensweisen, aber auch in einer Reizbarkeit und Enthemmung äußert.
Diese Agitation ist häufig schwer zu behandeln und belastet die Umgebung massiv.
Häufig werden Antipsychotika verordnet, obwohl deren Wirksamkeit umstritten ist
und auch deren Sicherheit kürzlich durch Studien infrage gestellt wurde, da deren Gabe
mit einem erhöhtem Sterberisiko der Patienten in Verbindung gebracht wird.
Citalopram, ein Antidepressivum aus der Gruppe der selektiven Serotonin-Wiederaufnahmehemmer (SSRI), wird bei älteren Personen teilweise als Alternative verwendet. Es gibt
bisher aber nur begrenzt Hinweise für seine Wirksamkeit und Sicherheit.
Die S3-Leitlinie „Demenzen“ weist auf die schwache Wirksamkeit von Citalopram bei
agitiertem Verhalten von Demenzkranken hin. Gemäß der Leitlinie kann ein Behandlungsversuch dennoch gerechtfertigt sein. Es handelt sich jedoch um eine Off-Label-Behandlung.
Die neue Studie
Das primäre Ziel der „Citalopram for Agitation in Alzheimers Disease Study“ (CitAD) (3)
ist es, die Wirksamkeit von Citalopram bei Patienten mit einer Alzheimer-Erkrankung
(ohne eine majore Depression) bei der Behandlung der Agitation zu beurteilen. Die CitADStudie wurde als multizentrische, randomisierte, placebokontrollierte, doppelblinde Parallelgruppenstudie durchgeführt. Insgesamt nahmen 186 Patienten aus acht US-amerikanischen und kanadischen akademischen Zentren teil. Die Teilnehmer waren durchschnittlich
78 Jahre alt, 46 persen waren Frauen, 65 persen waren weiß und nichtspanischer Abstammung. Die
meisten Teilnehmer nahmen ein Antidementivum ein: über zwei Drittel einen Cholinesterasehemmer und über 40 persen Memantin.
Bei den Probanden bestand seit durchschnittlich fünf Jahren die Diagnose Demenz. Zur
Diagnosestellung wurden die Kriterien des National Institute of Neurological plus Communication Disorders plus Stroke-Alzheimer Disease plus Related Disorders Association
angewandt. Eingeschlossen wurden Patienten mit einem Punktwert zwischen 5 und 28 im
Mini-Mental-Status-Test (MMST; english: Mini Mental State Examination, MMSE), bei
denen eine klinisch signifikante Unruhe mit Hilfe des Neuropsychiatrischen Inventars ermittelt werden musste und bei denen ein Arzt eine Medikamentengabe für angemessen
hielt.
Über 90 persen der Studienteilnehmer schlossen den neunwöchigen Versuch ab. Citalopram
wurde, sofern verträglich, von einer Tagesdosis von 10 mg innerhalb von drei Wochen auf
30 mg erhöht. Insgesamt erhielten 78 persen der Teilnehmer 30 mg und 15 persen 20 mg Citalopram täglich.

Untuk kurangi dampak samping, dokter Anda kebanyakan akan memberikan dosis yang rendah di awal pemanfaatan dan meningkatkannya secara berkala. Selalu ikuti arahan dokter untuk hasil yang optimal. Lebih baik kecuali obat ini dikonsumsi terhadap jam yang serupa setiap hari.

Tetap mengonsumsi obat ini sesuai dengan jangka selagi yang ditentukan biarpun suasana kebugaran Anda telah membaik. Jangan hentikan pemanfaatan tanpa berikan menyadari dokter karena dapat menimbulkan dampak samping. Kemungkinan Anda akan mengalami mood swings, sakit kepala, kelelahan, perubahan selagi tidur, dan menjadi disengat sehabis mengonsumsi obat ini.
